may be made by calling the office during the practice hours listed above.
If you have an urgent problem, please call us as early as possible
and we will make every effort to schedule you as soon as possible
with your physician or the first available physician.
If you are unable to keep a scheduled appointment, please let us know
as soon as you can, in order that we may give that time to another
patient.
On your first visit to the office, you will be asked to complete some
forms and we request that you come fifteen (15) minutes ahead of your
scheduled time to do so. Please bring the following items with you:
- Insurance card (s)
- Any records, tests, or films pertaining to the reason for your visit
- Any
pre-authorization forms or referral forms if required by your
insurance company - The referral from your primary care physician if you are a Medicaid or HMO patient
- Our
patient information form you may print this form from our
website and bring it with you. Please complete ALL information. - Your Copay
- If you are uninsured, please call us to make payment arrangements before your visit
We accept all major credit cards and we will be glad to accept your payment by phone.
We make every effort to see our patients in a timely manner, however, emergencies do arise from time to time and we ask for your understanding. Please provide us with a number where we can reach you on the day of your appointment. In the event the physician will be late or unable to come to the office, we make every effort to contact our patients and inform you of the situation.
If you need ambulance or wheelchair transportation to get to our office, please provide that information at the time you make the appointment.
If you are a nursing home resident or are located at an inpatient rehabilitation facility, please make sure we have your number at the facility so that we can contact you if necessary.
Fees and Payments: We accept most insurance plans. Please check to see if your insurance plan is accepted when you make your appointment. If you are in a plan that requires you to have a referral, please bring the referral with you or have it sent to us by your primary care physician (PCP).
Please bring your insurance card with you for your visit as it has helpful information our billing department often needs. Co-payments are due at time of service.
For patients without insurance, we expect payment at the time of your visit. If you need to make payment arrangements we will be happy to work with you.
We do accept VISA, Mastercard, Discover and American Express credit cards. For your convenience, payment may be made by phone.

Prescription Refills:  If you need a prescription refilled please call the office during normal working hours. This is the best time to call unless it's an emergency as your doctor is more likely to be available and your chart will also be available. With certain refills prudent medical care demands an office visit for reassessment of your problem.
Emergency Calls:  During the day the appropriate physician will be contacted. After office hours, the answering service will contact the on-call physician who will then respond to your call.
